Acne scar types in Singapore — a reader’s map

Ice-pick, boxcar, rolling, red marks and brown marks are not one treatment. A map of what people usually mean, and what a laser cannot invent.

“Acne scars” is the search. The photograph is usually a cheek in bathroom light. The advertisement is a single machine. The face, if you are allowed to look at it properly, is usually several problems standing together.

The old pages here asked, brightly, whether you were struggling with pores and acne scars — as if they were twins. They are neighbours. They are not twins.

Colour is not a scar

A flat red mark after a pimple is often post-inflammatory erythema. A flat brown mark is often PIH. Neither is a hole. Treating them as if they were CO2 problems is how people collect a second injury.

If the skin is still breaking out, scar talk is early. Active acne is a medical conversation, not a device tour. See teen and adult acne in this climate.

Ice-pick scars

These are narrow, steep, as if a pencil went in. They photograph badly and they do not spread out for a laser the way a rolling scar sometimes can. You will hear about chemical reconstruction of the surface — TCA CROSS — more often than about a gentle toning pass. That does not make TCA a homework project. It makes the geometry honest.

Boxcar scars

Broader, with edges, like a shallow punch. Some are shallow enough that fractional lasers are discussed. Some are not. The edge is the story. A brochure that shows a pore and calls it a boxcar is not helping you.

Rolling scars

These are wider, softer, and often tethered underneath by fibrous bands. People describe a shadow that moves when they smile. You will hear the word subcision — a release of those bands — sometimes before anyone warms up a laser. Laser alone on a tethered roll can be an expensive polish.

Hypertrophic scars and keloids

Raised scars are a different family. They are not “the opposite of ice-pick” in a way that invites the same menu. If you form thick scars, say so early. It changes what a cautious doctor will offer, and it should.

What lasers are for, in ordinary speech

Pico is often pitched for leftover colour and mild texture. Fractional non-ablative devices are pitched for a middle path. Fractional CO2 is pitched for deeper atrophic change, with a week that does not look like a lunch-hour. Radiofrequency microneedling sits in the same conversations; see microneedling versus RF microneedling.

Session counts you will hear — three to eight, a month apart — are not a covenant. They are a way of saying collagen is slow. Anyone who sells a single pass as a new face is selling a photograph.

Skin tone is not a footnote

Fitzpatrick numbering is imperfect and still useful. Lighter skin, in general, has more room for heat. Medium and darker skin, which is most of this city, collects PIH the way white linen collects tea. The Dermatological Society of Singapore’s patient pages are dry on this point: infection, colour change and scarring are real complications, not small print.

A plan that never mentions pigment risk is a plan for a conference in another country.

Cost, downtime and the calendar

People ask, reasonably, what a course costs. Observed 2026 menus in Singapore often put gentler lasers in the hundreds per session and deeper resurfacing higher, sometimes much higher once combination work is counted. Our cost essay is a map of bands, not a quote.

Downtime is a work question. A teacher, a flight attendant, a lawyer with a hearing on Thursday — these are not “social downtime” in a brochure sense. They are the week. Read the aftercare week before you book the Friday slot because it was free.

How to keep the appointment honest

Ask which scar types they see. Ask what they would not laser. Ask whether colour and texture will be sequenced. Ask who does the work — doctor, or a title that evaporates when you check the SMC register. Ask for the plan in writing.

Then go home. The scars will still be there in the morning. The package may not need to be.

How to look at your own face without a filter

Stand under a single overhead light and smile, then rest. Rolling scars often announce themselves as a moving shadow. Boxcars stay put with an edge. Ice-picks do not become more obvious when you grin; they were already obvious. Colour that is only colour will not catch a sideways light the way a dent will.

Do this before you sit down so you have language. “The shadow moves when I smile” is a gift to a good doctor. “I hate my scars” is a mood.

Combination plans are not upselling by default

Subcision plus a laser, or TCA plus something gentler for colour, can be a sequence rather than a bundle. The test is whether each step has a reason that uses a scar word. If the second device is added because the package looks neater, that is a sale. If it is added because a tether will not release for light alone, that is a plan.

Ask to see, on your face, where they would not treat. Ask how they will photograph you so the next visit is comparable. Ask what they will do if session three looks like session one.

Work, race week, and other calendars

A teacher’s face is public on Monday. A waiter’s face is public every evening. A lawyer’s face is public on a hearing day. Downtime is not “social.” Name the calendar. A conservative non-ablative pass may be the adult choice even if a louder laser would photograph better in a brochure.

If you form thick scars, say it twice. If a cousin needed keloid care, say that too. Family stories are data in this room.

Lighting you will actually live with

Clinic lights are a courtesy. Your life is a fluorescent MRT, a phone camera at dinner, and a bathroom that cannot decide between warm and cold. Ask to see the plan against those three, not against a ring light. If a type of scar is only “visible” in the room’s favourite lamp, you are buying a photograph, not a change you will notice on the train.

Take one photograph a month in the same spot. Do not take thirty in different toilets and call the set data. Scars move slowly. So should your judgement. If a room wants a decision after one comparison slide, take the slide home on paper and look at it beside your own month of pictures.

A useful question, asked twice: which type on my face is the one they are actually treating, and which type is being named so the menu sounds complete? The first answer should be a noun. The second should make you pause.

Questions people ask

Can laser remove acne scars completely?
Complete removal is rarely what doctors describe for established atrophic scars. Plans talk about softening, not erasing, and often more than one method.
Is pico laser enough for deep scars?
Pico is commonly discussed for colour and mild texture. Deeper boxcar or tethered rolling scars are often discussed with other tools, sometimes first.
Why does Asian skin change the plan?
Higher melanin can mean a higher risk of new brown marks after heat or injury. Conservative settings and sun are not timid. They are the local weather.
