HIFU and energy lifting, without the facelift fantasy

The old site sold Ultherapy as a face-lift without surgery. The honest version is still useful: what ultrasound is asked to do, who it disappoints, and how a year actually feels.

The 2018 homepage advertised Ultherapy from eight hundred dollars and a sagging face as a problem with a quick fix. Quick is the word that has not improved with the devices.

HIFU — high-intensity focused ultrasound — and branded cousins such as Ultherapy send energy to deeper planes, including the layer surgeons talk about when they talk about a lift. The skin above is supposed to stay intact. Collagen is supposed to argue with gravity over the following months. That is the brochure. The brochure is not entirely wrong. It is incomplete.

Who these appointments are for, in clinic speech

People with mild to moderate looseness — an early jowl, a softer jaw, a neck that photographs older than the face — who want to stay out of a theatre. Often thirties to fifties. Not a person whose skin has already made a large migration. Not a person who needs a surgeon and is being offered a device because the device is in the room.

If the pitch is “non-surgical facelift” with a celebrity jawline, you are in marketing. Stay for the physics if you like. Leave the celebrity.

What the hour is like

Consent. Photographs. Sometimes numbing, sometimes not. A grid on the face. A handpiece that clicks. Discomfort that people describe as heat and a deep ache, worse along the bone. An hour or two. Then a commute that looks, to the world, like you went to lunch.

Results, if they arrive, arrive late — weeks, then a few months. That delay is why testimonials shot on day two are usually about the service, not the lift.

What it will not do

It will not copy a well-done surgical lift. It will not remove a heavy neck. It will not fix a hollow that is really a dark-circle shadow. It will not replace weight change or a dental plane. It will not make a package of threads unnecessary if threads were never the right tool either.

The old site also sold thread lifts and a “V-face.” Those are other conversations with other risks. Do not let a consultant braid them into one price because you nodded at the word sagging.

Cost and the yearly rhythm

Observed Singapore menus put ultrasound lifting somewhere between the high hundreds and a few thousand a session, depending on device, area and who is holding it. Maintenance once a year is a sentence you will hear. So is “package of three.” Ask what they would do if the first session does nothing you can see in a photograph at month four. See costs.

Safety is a person, not a logo

Burns, unexpected fat loss, nerve irritation — uncommon in careful hands, not mythical. Who holds the device matters more than the brand on the stand. Check the register and the premises. Ask how many of these they did last month, not last decade.

Thin faces should ask about fat. A lift that also carves is a surprise some people do not want.

Questions

What layer are you aiming at? Why this device and not RF? What does a non-responder look like? What will I feel on the MRT home? If I have a wedding in six weeks, is this the wrong calendar?

Energy can be a modest ally. It cannot be a time machine. The people who leave happiest are usually the people who were not promised a new decade.

Pain, which is allowed to be interesting

People under-describe HIFU pain because they do not want to look fragile. The sensation along the jaw and near the mouth can be sharp. Numbing helps some and barely others. If you cannot sit still for ninety minutes of clicks, say so. A rushed map is how edges get missed and how a brow looks surprised.

Ask whether they pause for nerves. Ask what they do if you feel electric pain that is not the usual ache. “Power through” is not a protocol.

Photographs at month three

If they will not take a baseline in the same light, they are not planning to be judged. Take your own, same bathroom, same hour, no smile, at week zero and week twelve. Show them. A good room likes this. A shop does not.

If nothing you can see has changed at month four, you are allowed to stop. Sunk cost is how packages finish themselves.

Neck, brow, and the rest of the brochure

Necks are harder. Brows can look odd if energy is placed like a wish. Bodies are a different consent. Do not let a face package wander onto a stomach because the device was free in the last ten minutes. Each area has a fat conversation and a nerve conversation. Ask both.

Combining HIFU with needles and threads

Rooms love a “makeover month.” A thread plus HIFU plus a filler is a lot of swelling and a lot of ways to blame the wrong tool. If you want more than one, sequence across months and let each have a photograph. A single Saturday is a party, not a plan.

Ask who is legally responsible if a brow looks wrong. Get a name.

Weight, teeth and the rest of the face

A lift device will not argue with a molar you are about to lose or a weight you are in the middle of changing. Tell them. A jawline that is really a dental plane will disappoint a device and then disappoint you.

What a non-responder can still do

If month four is unchanged, you may stop, you may see a surgeon, you may change the goal. You do not have to buy the next device in the corridor. A good room will say this before you pay. A shop will say it after the package is empty. Ask which sentence they prefer, today, out loud.

Pain, numbness, and the story you tell at dinner

HIFU is often sold as lunchtime. Lunchtime does not include a jaw that feels odd, a brow that looks surprised, or a week of explaining why you do not want spicy food. Ask what sensation is expected, what sensation is a reason to call, and whether anyone will answer after 7pm.

If you have dental work, TMJ pain, or a habit of clenching through the day, say so before the handpiece. Energy near a joint is not a lifestyle choice. It is a conversation a careful room should want to have.

Photograph the face at rest, not in a smile you do not use on the MRT. Lifting claims love animation. Your life is mostly a resting face in bad light. Judge the result there, at six weeks, not in the mirror at day three when swelling is still doing unpaid work.

Questions people ask

Is HIFU a facelift?
No. It is an energy treatment aimed at deeper support layers. A surgical lift is a different operation with a different recovery and a different result.
How long do HIFU results last?
Clinics often quote many months to a year or more, then talk about maintenance. Bodies and operators differ, and the quote is not a warranty.
Does HIFU have downtime?
Many people are told to expect little visible downtime — perhaps swelling or tenderness for a day or two. Jawline numbness or unexpected swelling should be reported, not waited out in silence.
