
Excessive uncontrollable sweating on the underarm or palms may cause severe social, psychological and professional distress. Excessive sweating, or hyperhidrosis, is very common. In fact, most of you know someone who constantly has sweaty underarms or palms.
Thankfully, you can be treated. In my other article titled “hyperhidrosis”, we talked about different treatment options for this problem. I will only focus on Botox® injection here.
How does it work?
Botulinum toxin is a natural, purified protein with the ability to temporarily block the secretion of the chemical that is responsible for “turning on” the body’s sweat glands. By blocking or interrupting, it “turns off” sweating at the area where it has been injected.
How is it done?
A fine needle is used to inject tiny amounts of Botox just under the skin throughout the area of excessive sweating, usually 1 to 2 cm apart. Multiple injections are usually needed. It can be done on the same day visit.*
Where does it work best*?
Botox is best used for underarm and palms. It can also be used for sweaty face but the potential side effects when used on the face are higher. Sweaty soles can also be treated with Botox injection but it is not as effective.
Painful*?
Pain is minimal. Most people tolerate it very well. A numbing cream is given prior to injection to minimize pain. The needles we use to inject are extremely tiny so you can barely feel them. You may need up to 15 to 20 injections especially in the underarm, to yield the best results.
How effective is Botox?
Botox has been shown to result in 82 – 87% decrease in sweating if used in underarm, and 80 – 90% if used on the palms. However, only 50% of patients are satisfied with the results if used on the soles or face.
How quickly does it work?
You may notice some results approximately 2 to 4 days after treatment with full effects usually noted within 2 weeks.*
How long does the effect last?
It generally lasts for 4 to 6 months, after which time you will need repeat injections if necessary.*
Is it safe*?
It is safe. Statistics from published research studies have shown that repeated treatment with Botox is safe and effective for excessive sweating and consistently results in meaningful and long-lasting improvements. However, you need to be aware of potential complications although they are rare.
Some bleeding may occur during the injection but it can be stopped quickly by pressure.
You may experience some redness, swelling and bruising at the injection sites but they will disappear in less than a week.
If used on palms, it can cause temporary pain and weakness in the hands.
If used on face, you may experience asymmetry of the face especially on the forehead. Such asymmetry, however, is always temporary.
If used on soles, it can be more painful than other areas.
What happens after the treatment*?
Follow up is usually arranged in 1 to 2 weeks. We will need to assess the results and potential side effects, and to “touch-up” any sweating areas that may have been missed during the first round of injections.
You will need repeated injections after about 4 to 6 months.
